BackgroundThe evaluation of pain remains one of the most difficult challenges that healthcare practitioners face. Chronic pain appears to affect more than 35% of the population in the West, and indeed, pain is the most common reason patients seek medical care. Despite its ubiquity, studies in the last decades reveal that many patients feel their pain is dismissed by healthcare practitioners and that, as a result, they are denied proper medical care. Buchman, Ho, and Goldberg (J Bioethic Inq 14:31-42, 2017) (...) point to this phenomenon as a form of “epistemic injustice”: an unfair and harmful downgrading of credibility affecting some individuals and groups, which prevents them from receiving appropriate and beneficial medical care.MethodsBy exploring the existing literature on this downgrading of patients’ credibility written by healthcare professionals and scholars in medical humanities, I identify and examine the reasons patients are often not believed about their pain and why healthcare is too-often unhelpful or hurtful to people presenting with chronic pain. I also explore to what extent it is possible to forge an alternative epistemological model.ResultsI suggest that most of the causes of this downgrading of patient’s credibility result from either the difficulty in communicating pain or the widespread belief that pathology is always the result of objective tissue damage. I examine whether pain has to be effectively communicated and have an objective cause in order for it to be deemed credible. In the end, I argue that in the case of pain, both communication and objectivity are highly problematic.ConclusionsI conclude by suggesting that, although alternative epistemological models might be impossible to build, believing patients has both moral and clinical benefits, and this warrants further research. Terraforming is a process of planetary engineering by which the extant environment of a planetary body is transformed into an environment capable of supporting human inhabitants. The question I would like to consider in this paper is whether there is any reason to believe that the terraforming of another planet—for instance, the terraforming of Mars—is morally problematic. This article investigates the naturalisation of the Bible. Three voices are of special importance in the narrative presented in this article; they are Aristotle, Rene Descartes and Baruc Spinoza. This article will investigate the scientific method and metaphysics espoused by each of the three scholars, thereby highlighting changes in scientific method and metaphysics that lead to the naturalisation of the Bible. Firstly, Aristotle pioneered a scientific method that would dominate for centuries, as well as a highly influential metaphysics. Secondly, Descartes, (...) witnessing the horrors of the Thirty Years War and seeing first-hand the new discoveries that brought about the scientific revolution, reacted against Aristotle’s metaphysics. Ironically he then used Aristotle’s scientific method to provide a foundation for the new science resulting in Descartes’s famous dualism. Thirdly, Spinoza, equally horrified by the amount of religious violence of his time, reacts against Descartes’s dualism, providing scholars with a monist metaphysics that would contribute greatly to the naturalisation of the Bible. Theologians are used to pointing the finger at European continental postmodernism when dealing with modern relativism. This article addresses a problem that is seldom highlighted within theology: modern relativism is the result of a series of epistemological discussions that took place during the early Enlightenment between scholars such as Rene Descartes, John Locke and Immanuel Kant. They were reacting, in part, to Aristotle’s metaphysics and logic. When the whole picture unravels, one immediately sees that modern relativism is deeply ingrained in (...) Western thought. In other words, modern relativism will not gather dust after the demise of postmodernism. To the contrary, this article would argue that modern relativism will continue to pose serious challenges to reformed churches in future. Pastors who want to engage with Western audiences will benefit from being made aware of this. Hopefully this will encourage theologians to re-evaluate the relevancy of reformed theological constructs in societies that are deeply steeped in relativist thought. (shrink)

In a recent article in this journal, Del Mar offered two main criticisms of Marmor’s account of social conventions. The first took issue with Marmor’s claim that the constitutive rules of games and kindred social practices determine in an objective way their central aims and values; the second charged Marmor with scanting the historical context in which conventions do their important normative work in shaping the goals of games. I argue that Del Mar’s criticism of Marmor’s account of the normative (...) centrality and force of constitutive rules in games and the like fails, but that his criticism faulting Marmor for giving short shrift to the normative work conventions do in these social practices is on the mark. So while I reject Del Mar’s claim that a closer look at the social and historical contexts in which the conventions of games and the like carry out their normative tasks undermines Marmor’s account of constitutive rules, I think his argument that conventions play a far more important, even if supplementary, role in shaping our understanding of and participation in these social practices than Marmor allows is persuasive. A Dutch 'folk theorem' holds that 'from exchange it comes to tears'. This seems to contradict the basic idea found in economics that exchange and trade can make both sides better off. We show that the 'folk theorem' has a better theoretical foundation than sometimes thought, as it is vindicated by the equilibrium of an exchange game with two-sided asymmetric information. We, then, explain the practical value of such 'folk wisdom' in the real world by showing why players might be (...) unlikely to learn such an equilibrium strategy. (shrink)

Explaining both who God is and how we can relate to him, this thought-provoking work seeks to transform and enrich the Christian understanding of God.

Educational efforts in bioethics are prevalent, but little is known about their efficacy. Although previous work indicates that courses in bioethics have a demonstrable effect on medical students, it has not examined their effect on health care professionals. In this report, we describe a study designed to investigate the effect of bioethics education on health care professionals. At the Intensive Bioethics Course, a six-day course held annually at Georgetown University, we administered a questionnaire requiring open-ended responses to vignettes both before (...) and after the course. Following the course, respondents defended their responses more carefully and articulated their thoughts more clearly. In addition, after the course respondents seemed to have a more subtle understanding of the relevant issues in the cases and applied theory to these cases more frequently. These findings help to formulate an understanding of the effect of bioethics education on health care professionals. (shrink)

Ashley and Stack couple their claim that Hume holds a logical-construction theory with the remarkable suggestion that, so understood, his views yield "... at least a recognizable facsimile of the identity most of us believe in." The highly implausible suggestion that the non-philosopher regards his self as a logical construct should be enough to provide a motive for re-examining the arguments Ashley and Stack offer for their interpretation. These arguments make use of the distinction Hume develops between perfect and imperfect (...) identity and lead, first of all, to the conclusion that, contrary to some interpretations, Hume does not refuse the self all identity, but only perfect identity, that is, identity defined as the "idea of an object, that remains invariable and uninterrupted through a supposed variation of time." Finding the self to be not such an object, Hume says it cannot be said to have such identity. It can, however, be said to have imperfect identity, constituted by the "uninterrupted progress of thought" over a succession of objects, namely our perceptions. (shrink)

The object of this paper is to argue that the relationship between intentions and meaning has been misconstrued by some influential recent theories of meaning. The theories I have in mind derive from earlier work by H. P. Grice, but have undergone extensive development and modification in the hands of Grice himself,, Stephen Schiffer, Jonathan Bennett and others. There have been, during much the same period, developments of Austin’s work on speech acts in which the same Gricean influence is unmistakable; (...) indeed, such influence has affected interpretations of Austin’s work itself, by such writers as Strawson, Searle, and Vendler. Women are making a substantial impact on the employment market, both in terms of overall numbers as well as by appointment to male-dominated organizational roles. Research on women in leadership positions within organizations has concentrated on two main foci. Firstly, the identification of relevant individual and organizational characteristics and secondly, on the impact of these variables on the women in management roles. This paper presents the findings from a series of studies in relation to these broad dimensions.
